What is Connections Housing?

Like most cities in our country, San Diego grapples with how best to help people who live on our streets, while also ensuring quality of life and safety for businesses and residents in areas affected by homelessness.

In 2008, the San Diego City Council, with the support of the Mayor, the San Diego Housing Commission, and CCDC (now Civic San Diego) initiated a vision for a one-stop homeless services center. In 2010, a volunteer citizen’s panel, along with the approval of the San Diego City Council, selected PATH as the lead agency for the project.

PATH San Diego's Connections Housing is a partnership which brings a variety of agencies together under one roof to meet the many needs of the downtown homeless population.

The word "connections" is symbolic of the collaborative and community-based nature of the project. PATH is the lead agency providing the services and operations for Connections Housing. The building was developed by Affirmed Housing Group and PATH Ventures. Family Health Centers of San Diego is also a founding partner as well as the health center operator.

    For over 30 years, PATH has worked to address homelessness in California through a combination of services leading to housing, including rapid re-housing assistance, strategic outreach, interim housing, permanent supportive housing development, employment and supportive services for the most vulnerable populations. PATH serves as the lead service agency and building operator of Connections Housing. More information: www.epath.org

     

    Family Health Centers of San Diego - www.fhcsd.org

    Family Health Centers of San Diego (FHCSD) is the second largest federally qualified health center in the country and an integral part of San Diego's healthcare safety net. Since 1970, its mission has been to provide caring, affordable, high quality healthcare to anyone in need, with a special commitment to uninsured, low income, and medically underserved families. Family Health Centers of San Diego operates thirty locations throughout the County of San Diego, including 15 primary care clinics, 3 dental clinics, an HIV clinic, and 3 mobile medical units which provide healthcare services at approximately 70 community sites. In 2010, Family Health Centers of San Diego cared for more than 161,000 individuals through more than 598,000 patient encounters. The organization is a leader in the development and delivery of innovative programs that effectively address the specific needs of San Diego's underserved communities. As the lead agency for the San Diego Health Care for the Homeless (HCH) project since 1988, FHCSD has over 20 years of extensive experience and expertise in providing primary health care, mental health, vision, dental care services to the San Diego homeless population.

    Affirmed Housing Group is dedicated to improving and sustaining the viability of California communities through the development of well designed and professionally managed affordable housing. Affirmed Housing Group has extensive experience in residential development. Areas of expertise include site selection, engineering, architecture, construction, public financing, low-income housing tax credit acquisition and tax-exempt bond financing. Since 1994, Affirmed Housing Group has secured over $661 million dollars in financing to create over 2,224 housing units.

     

    PATH Ventures - www.pathventures.org

    PATH Ventures (PV) seeks to improve individual lives and communities by increasing quality affordable housing and creating opportunities for economic, civic and personal empowerment. PV develops permanent supportive housing for homeless and very-low-income individuals and families. PV has completed 11 housing communities and nearly 700 units, with more in the pipeline.
